Pretty much what Gunsmoke stated. We prefer metal rings which go around both cock and balls, but on occasion will use one which fits just the penis.

A bit of a variation is the medical "tension ring" used with a vacuum pump for treatment of ED fits over the penis alone, like what Gunsmoke described. Tension rings are extremely tight and rather uncomfortable even though they work quite well for the purpose designed.

Our BG 001 & JBoa are meant to me worn only on the Penis. The JBoa is adjustable in size so the tension can be adjusted. The BeauGosse is nice and tight (one size fits all) but also wears comfortable due to the design and chosen material.

Is actually quite easy with just a little practice. Just place the ring against the scrotum and gently pull the skin through the ring, and push one ball through the ring. It depends, on how big it is. I've seen some that are meant to go around the balls, and cock, as well as just the cock. I think it partially comes down to, can you get it around both or just one....

depends on the style of cock ring. Some just go around the shaft, while others go around the entire base, both penis and balls. The latter one can be a bit tricky to get on sometimes, and you definitely have to be soft to get it on.

I wear two rings. The first is a 2" metal ring that I wear around my penis and scrotum. I put it on exactly like Checkmate describes.
The other,a 1.5" metal ring,I wear around my scrotum. The setup is like a three ring clock and ball cage.
I like metal rings because I enjoy the tight feel and the "cool" metal. I find elastic rings to be too tight.

It really is based on discretion and comfort. Most cock rings will specify if it can be worn with the balls or not. If you can't tell or it doesn't specify, look for sizes. Normally the sizes will be in inches. Look for the larger ones if you're looking to have them worn under the balls, if not the smaller ones should be okay worn at the base of the penis.
