Only in porn, but then again, porn isn't a necessarily representative of what consenting people do in real life. Sorry I didn't see your note before. For what its worth my fingernails are nearly non-existent - I've always kept them very short. My wife's nails a relatively small for a woman - but long enough to paintable - and long enough to be a pain in the ass if she's not careful!

I always try to keep my nails trimmed/well filed as anal fingers during sex w/ my partner is something we both enjoy. Hygiene is number one priority, especially when you later plan on switching between the orifices - just DON'T. This is absolutely true. A finger massage is the best way to stimulate and relax the exterior anal sphincter - and short of a lube shooter the best way to get the lube inside and past the secondary sphincter.

From there a second finger may or may not be the best choice. My wife much prefers a smallish (1.25") vibrating silicone butt plug. The vibrations are important for helping relax things.

We may go on from there to vaginal play and some g-spot gushing because the plug adds upward pressure on anything inserted vaginally - including yours truly.

If we are going to progress to full on anal penetration then I will work the plug in-and-out - reapplying lube frequently - until I am confident that she is relaxed enough for penetration by me. Of course I check with her to confirm her readiness.

Although I do have one little note - before progressing to full anal penetration, working it with a plug might not be the best and the most comfortable way. Plugs are not meant for thrusting - at least the majority of them - so they're often awkward to hold on to during the thrusting motion, and the gradual size-up/size-down with the neck might prove ineffective after a couple of thrusts.

My bet is anal probes - much more convenient for thrusting. I especially love those probes that are shaped like anal beads, but with no spaces between the beads. Magnificent for a warm-up!

GingerAnn is right - butt plugs can be awkward - however, we use the Tantus B-Bomb which is made with a very smooth silicone finish and has a hole in it for inserting a vibe. I use a very inexpensive 'pencil' type vibe that uses 2-AA batteries and is about 6" long. The part that extends away from the plug makes a great handle!

You are also correct that tapered dildos or vibrators are great choices - and for anal play slippery smooth silicone is preferable to the matte finish options that have too much drag - even when copious amounts of lube are used.
