For me, this has definitely happened with fingers, but never with a tongue. I prefer the whole area to be stimulated before direct stimulation on the clit.

Some people have extremely sensitive clits and they can't stand being touched on the glans until they're about to cum. This may be the case with your wife. Try starting off licking around her clit or on the hood/labia/vagina and when she's worked up enough, start gently on her clit.

All about variety and depends on how stimulated (if at all) I am before they start

If I had to choose I think I like a slow build of pressure and speed (so starting off very light), right up until I can't handle it any more and then back way off, more gentle, more variety in stimulation, and then building again
