The Form 4 by JimmyJane comes packaged in a white box with a picture and product description. Inside the box comes the toy itself, instruction manual and warranty information, charging base and cable. Form 4 comes with a 3 year limited warranty.

Form 4 is made of smooth silicone. I decided to go with the slate version so mine is black colored silicone. There is a little drag on the silicone material and it does collect lint. It has seams along both sides dividing the toy in half. The shape of Form 4 is interesting. Before I saw it in person I thought it was more of a round, circular shape around. It's actually somewhat flattened. The base of the toy is larger than the top, looking somewhat like a flattened bowling pin. The base measures about 1 3/4 inches in diameter, and if you turn it to it's side it measures about 1 1/2 inches. The top is more circular and measures 1 1/2 no matter which side you measure from. The total length of the toy is 5 1/4 inches long. Form 4 is completely waterproof and allows for easy cleanup with soap and warm water or toy wipes. Since it's made of silicone, only water based lubes should be used with it. Silicone is rated 10 on the Eden Fantasys safety scale so it's a great choice. The toy charges via the charging connector at the base of the toy. Just sit the toy on the charging base so that the charging connector makes contact. You know when it's positioned correctly and charging because a light comes on the front of the toy. To gain full charge for the first time it should be charged for 8 hours, every time after that it only takes about 2 hours. You can just leave it on the charging base when not in use.

Form 4 operates with a 3 button panel located at the base of the toy. It will not remember what setting you were using last. The top button shows a +, directly below that is a -, and below that is the pattern button ~. The light on the toy will flash when you press a button. To turn it on just press the + button. Change the vibration intensity by pressing the + or - buttons. If you're like me and want max intensity you can hold in the + button longer to get the maximum intensity. To change patterns press the up or down arrow. There is no way to just cycle through all the patterns, once you reach the beginning of the patterns you must press the down arrow to go back to the end of the patterns.

There are 5 vibration intensities and 4 patterns including steady vibration. The first pattern is steady vibration, the second is a roller coaster/escalating pattern and third and forth are slow and fast pulsing patterns. You can turn Form 4 off by holding in the - button until it turns off. The vibrations can be felt in the base but are strongest at the smaller end. I rate this toy 4 vrooms and 2 bees. It's quiet but at the loudest setting it can probably be heard through the covers but not a closed door. Form 4 can be used vaginally or externally for clitoral stimulation. Because the toy has no flared base it's not safe for anal play.
